HWSA OVER 30 WOMEN'S LEAGUE GAME REPORT

While we appreciate your diligence in completing this form, the printed game report form (returned by the referee) is still the official record of game results. However, submitting this form does meet the requirement that you notify the over 30 commissioner of the game results by the day following each game.

The paper game report will be used to verify results submitted here. In order to validate theses submissions, please choose a password the first time you complete the form. This password will be used to validate subsequent game reports, so don't lose it.

ATTENTION: Red card for violent conduct (fighting) draws automatic three game suspension effective immediately (minimum penalty), plus a fine, and an appearance before the league's Discipline and Protest Committee. Other red card infractions involve greater or lesser penalties.  Do not risk forfeiting future games by playing an ineligible player -- politely check with referee regarding type of
offense recorded on game report sheet, and review HWSA General Rules to determine penalties. If there is any question about length of suspension, please contact the Over 30 League Commissioner.
